Veterinary Passport for Pets
A veterinary passport for pets is a document that contains essential information about the animal and its owner. The passport includes the pet’s name, date of birth, breed, sex, color, distinguishing features, and the owner's contact details. If the animal is microchipped, the chip number is recorded and an identification sticker is attached.
Important: this passport is an internal document and does not grant permission to cross international borders. To travel outside Ukraine, an official international veterinary passport must be issued exclusively by state veterinary authorities.
Purpose: The veterinary passport is used to maintain a record of preventive measures and procedures. It includes information on vaccinations (including rabies), deworming, treatments against ectoparasites, microchipping, and other veterinary procedures.
Main functions:
- Storage of identification data of the pet and its owner.
- Recording of vaccinations and preventive treatments.
- Confirmation of the animal’s veterinary status within the country.
- Use for internal registration and obtaining veterinary certificates.
Issuance details: The passport is issued by a veterinarian during the first visit or vaccination and is maintained throughout the animal’s life. Records of vaccinations and procedures are confirmed by the veterinarian’s signature and clinic stamp.
Usage: The document is used for internal veterinary records, obtaining certificates, participation in exhibitions, and confirmation of performed procedures.
Benefits:
- Keeps a complete history of vaccinations and veterinary procedures.
- Simplifies obtaining certificates and passing inspections within the country.
- Is an important document for responsible pet care.
